Evidence of Insurability (EOI) may be required –-
The following are some general guidelines to keep in mind:
- New employees electing supplemental life insurance coverage for the first time will need to provide EOI if the coverage amount exceeds three times pay or $1,000,000. You will also have to provide EOI if the amount of your combined basic and supplemental life insurance totals more than $2 million. Spouse insurance in excess of $50,000 will require EOI.
- If you currently purchase supplemental or spouse life insurance, amounts which exceed your current coverage levels may require EOI.
- If you were eligible to elect supplemental life or spouse life insurance in the past and declined coverage, EOI will be required for all amounts elected.

If EOI is required, coverage over the limits will not be effective until it is approved by the insurance company. Coverage will be reduced to the next lower option until it is approved.
