For New Hires and Newly Eligible Employees
If you’re a new employee, or now meet the Company’s eligibility requirements for the first time, you must complete the enrollment process within 31 days of the date you’re eligible for benefits. Your benefits will be retroactive to the date you became benefits eligible.
If you don’t enroll and don’t waive coverage, you will receive the following default benefits:
Medical coverage under the CIGNA Basic PPO for yourself only; prescription drug coverage through CVS/caremark.
Company-paid basic life insurance and basic accidental death & dismemberment insurance equal to two times your annual pay, up to a maximum benefit of $1,000,000. (Short-hour and term-of-project short-hour employees receive coverage of one times annual pay).
Company-paid long-term disability (LTD) insurance equal to 50% of base pay, up to a maximum of $20,833 per month. (Term-of-project full-time and term-of-project short-hour employees are not eligible for this coverage.)
